**What is a Home Care Packages Team Leader for Hume?**

Working as part of the Community Health and Wellbeing department the Home Care Packages (HCP) Team Leader has responsibility to sustainably maintain the quality of the care management service as the HCP program continues to grow and respond to future requirements arising from the Aged Care Reforms.

**_ Duties of an HCP Team Leader might include but not limited to:_**
- Supervision of care advisors and hcp administration support staff
- Managing the day-to-day operations of the hcp program
- Surge support and coverage with ad hoc case management capacity
- Responding to new requirements introduced as part of continuing aged care reforms.
- Ensuring the provision of high quality services in accordance with funding and legislative requirements, including evidence of compliance with aged care quality standards.

**About You**

As an HCP Team Leader you will have demonstrable skills and qualifications including but not limited to:

- Leadership - creating and enhancing positive team culture through coaching and empowering individuals.
- Change Management - Ability to lead and drive change along with implementing and monitoring continuous quality improvement
- Direct experience as a care advisor/case manager in home care packages (or relevant field i.e. aged and disability)
- Tertiary qualifications in Community Development, Nursing, Allied Health, Human Services, or related field (to meet requirements to undertake the care advisor/case manager role when required).
