We are seeking an experienced **Auslan Support Coordinator** for minimum 3 days a week. We have a vacancy within our Brisbane team.

We are a group of creative professionals dedicated to providing supports to an expanding client base all over Australia. Reporting to the CEO, you will be supported by a team of established Support Coordinators.

**About Signpedia**

Signpedia is a Deaf owned organisation that provides services to Deaf, hard of hearing, nonverbal clients, Auslan users and their families, who have funding through the NDIS.

Our head office is based in Sydney, and we have offices in Adelaide and Perth. We also support clients in Melbourne, Bendigo, Newcastle, Brisbane and Cairns.

**Key responsibilities**

Client (NDIS Participant) Support:

- Respond and action all client referrals in a timely manner.
- Conduct all required Assessments, Initial Support Assessment, Participant Risk Assessment, Environment Risk Assessment with the client.
- Complete all on-boarding tasks for new clients; Service Agreement, copy of NDIS Plan, connect with clients Plan Manager (if applicable), NDIS Consent Form, Participant Consent & Acknowledgement Form, set-up client profile on Visual Care System, notify invoice team & state team of new client.
- Develop a support plan based on their NDIS goals and the approved supports defines in their NDIS Plan that clarifies the actions to be taken and the outcomes to be achieved.
- Implement support plan by coordinating supports using a skill development and capacity building approach, including formal and informal supports, mainstream services, specialist services and NDIS funded supports.
- Assist the client connect with necessary service providers (example; speech therapy/physio).
- Empower the client and their family (if appropriate) by providing information about the range of support services available to them.
- Support the client in facilitating & enabling empowerment, choice & control, and the promotion of independence.
- Provide support based on developing skills, competencies, and resources to enable the client and their family to promote the client’s wellbeing and the development and family’s capacity to be self-managing.
- Prepare for NDIS reviews and meetings with LAC’s and other service providers.

Recording and Reporting
- Provide the NDIA timely and relevant progress reports reflecting progress towards achieving plan goals and recommendations about the appropriate type of support required in the next plan and supporting information.
- Maintain and update complete case notes and other records as outlined in the support coordination policy and maintain accurate records of hours of support provided to substantiate NDIS claiming.
- Maintain written program documentation outlined in the service policy, standard guidelines, consistent with the NDIS Act.
- Keep an up-to date timesheet as provided by Signpedia with accurate information.
- Manage and escalate appropriately any perceived or real conflict of interest in accordance with NDIA’s terms of business.
- Undertake other duties and functions as directed, commensurate with current level of skills and classification.

**We seek the following qualities**
- Fluency in Auslan.
- Tertiary qualifications in Social Work, Psychology, Allied Health or similar are valued.
- Certificate IV in Disability Services or similar and/or equivalent knowledge and experience in the provision of quality community services for people with disabilities.
- Excellent written, verbal, and non-verbal communication skills.
- Ability to adapt to different methods of communication style to suit people’s needs.
- Demonstrated skill in researching information and developing effective and innovative support options.
- High level of administrative skills, time management and efficient work practices.
- Excellent computer literacy skills.
- Able to be open-minded, flexible and explore creative ways to meet client’s individual support needs and difficulties.
- Demonstrated networking skills and ability to develop networks within the disability industry.
- Ability to understand the NDIS Price Guide and flexibility within budgets, the NDIS legislation and rules including provisions relating to reasonable and necessary supports and the role of the mainstream service system.
- Ability to work collaboratively with others and alone with mínimal supervision.
- Demonstrated ability to empathise with people with a disability and their families/carers.
- High standards of ethical and professional behaviour.
