**About the Role**

Are you an existing Care Coordinator who is enthusiastic and positive, and looking for a rewarding challenge?

As the Care Coordinator, you will assist participants and their families engage in our services to enable them to achieve their short and medium-term goals. You will develop relationships with the participants, families, Support Workers, and allied health professionals. You will have a clear understanding National Disability Insurance Scheme (NDIS).

Your key responsibilities as a Care Coordinator Includes:

- Providing expert advice to enquirers and current clients through home visits, and assessments
- Conducting client reviews to ensure an ongoing high level of service provision
- Assist NDIS participants to implement their NDIS Plans
- Building meaningful relationships with potential and existing clients and providing exceptional service to ensure customer-centric support is provided
- Working closely with the Business Development Manager, you will upskill staff, assist in staff matching, provide regular supervision, and develop and implement client care plans

**About You**:

- Experience working with culturally diverse individuals.
- Excellent administration skills and the ability to interpret information guides, processes, and policy documents.
- Ability to work in a very busy environment, work independently and as a part of a team
- Skills in relationship building, conflict resolution, and negotiation.
- Innovation and creativity to work alongside people to achieve their NDIS plan goals
- Understanding of disability service delivery principles and NDIS
- Experience developing and managing care/support plans
- COVID-19 Vaccination, WWCC and NDIS Worker Screening Check

We are an Approved Provider under the NDIS. We provide a range of support and services (SIL, Personal Care, Respite Care, Medication Assistance, Domestic Assistance, and many more) We also provide support with community participation and companionship, community Nursing, and Accommodation.

We aim to engage the Support Worker & the participant along with their family and friends when preparing the care plan to make sure the participant’s goals are aligned and understood by the Support Workers.

Pay: $70,000.00 - $85,000.00 per year
