Critical role in a rapidly expanding purpose led organisation
- Provide comprehensive care and support for Disability / Aged Care individuals

**Our client** is a purpose led organisation committed to providing high quality support to individuals living with disability and their families through a range of services throughout Western Suburbs of Sydney. The organisation delivers a range of targeted and flexible individualised support initiatives including Day programs, Community and Accommodation support services.

The
**Support Coordinators **play a critical role in identifying the needs of clients and their families and develop tailored support plans to maximise the independence of those living with disability and helping them live full lives - just like anyone else

**Responsibilities and Duties**:

- Working closely with clients and their families to identify their needs and develop personalised support plans to ensure they are receiving best solutions and appropriate services.
- Oversee client plans and support and empower them to understand their NDIS plans by promoting and fostering independent decision-making.
- Assists with the day-to-day operations of site/s including financial process and ensuring they are maintained to appropriate standards, regulations and legislations.
- Contribute to continuous improvement, strategic direction, planning, and service delivery to help maximise outcomes for clients.
- Reporting, documentation, and business administration requirements.
- Contribute to positive and collaborative relationships with relevant stakeholders, building a network of people as required.

**How you will make a difference**:

- Relevant qualifications in community and social service management or equivalent.
- Minimum 2 years’ experience in a similar role.
- Well-developed IT skills, organised and self-motivated.
- Effective verbal and written communication skills with a friendly and caring manner.
- Full working rights.
- Unrestricted NSW Driver’s licence.
- Current First Aid certificate, WWCC, NDIS Worker Check, National Police Check
