**About the Opportunity**:
The service is provided to residents of Darebin, Hume, Merri-bek, and Yarra in accordance with the Palliative Care Australia Standards 2005. The primary purpose of the role is to implement the MCMPC Comprehensive Palliative Care in Aged Care Initiative that seeks to strengthen access and outcomes to a palliative approach for Residential Aged Care Facility (RACF) residents and to build the capacity of the RACF workforce to provide palliative care and end of life care consistent with Victoria’s end of life and palliative care framework. Working with the existing MCM Aged Care Team and Palliative Care Leadership Team, you will lead a project designed to strengthen relationships with RACF’s within the Melbourne City Mission catchment by developing approaches to understand the unique barriers and enablers for palliative.

In consultation with the Senior Manager, project sponsor, and Aged Care Team, the project officer will implement the Comprehensive Palliative Care in Aged Care Initiative.

Responsibilities include:

- Developing and Implementing stakeholder engagement initiatives to quantify awareness and understanding of the palliative approach to care across catchment RACF’s
- Directly engaging with Residential Aged Care managers and their workforce to identify and describe barriers and enablers to palliative care, unique characteristics and environmental features per setting.
- Supporting the establishment of communication and collaboration between palliative care and RACF
- Utilising existing and / or developing and producing tailored resources in collaboration with stakeholders that strengthen the capacity of the aged care workforce to provide a palliative approach to care, including those which identify evidence-based models of learning and skills development supporting the effective and appropriate provision of palliative care in residential aged

**About you**:
We are looking for someone who:

- Excellent interpersonal and networking skills.
- Strong written and verbal skills.
- Satisfactory completion of safety screening including a National Police check, International Police check (if required), current Victorian Drivers Licence, and the right to work in Australia.
- Palliative care experience.
- Continuous Quality Improvement skills and experience.

**Essential**:

- Tertiary qualification in Nursing or another relevant clinical field.
- Post graduate qualification in palliative care, aged care or disability care.
- Demonstrated project management experience and skills.
- Demonstrated change management experience
- Experience working in or with residential aged care and/or disability sectors.
- Excellent stakeholder engagement skills.

**Screening Criteria**:

- National Police check
- Working with Children check
- Valid Victorian Drivers license
- Right to Work in Australia
- Up to date with COVID-19 Vaccinations or valid medical exemption.
