**What You'll Do**:
- Client Sign up and onboarding.
- Conduct care plan reviews, ensuring clients receive tailored care.
- Support Level 1 and 2 HCP Clients with ongoing Care Planning and Budgeting.
- Support our Case Management team regionally and be a brand ambassador during client visits.
**What We're Looking For**:
- Strong communication and interpersonal skills.
- Proactive and organised team player.
- Knowledge of Health Care/Aged Care systems.
- Administration and Customer Service
- Solid understanding of HCP budgeting.
- Minimum 1 year experience in HCP case management.
- Ability to handle difficult conversations with clients regarding change in circumstances and budget usage.
