ABOUT THE ROLE

As the Case Manager Team Leader for our Home Care Package Program, you will play a pivotal role in steering the day-to-day operations of the Case Management Team.

Your primary focus will be on fostering a culture of excellence, ensuring that our Case Managers operate at their best, delivering client-focused care.

To achieve this, you will maintain a small caseload, enabling you to stay abreast of the intricacies of the role and provide hands-on support when the team faces capacity challenges.

ey Responsibilities Include:

  • Lead and support a team of skilled case managers
  • Foster a culture of collaboration and professional growth
  • Maintain a small caseload to stay current and provide handson support
  • Conduct regular performance reviews and promote continuous improvement
  • Ensure highquality care plans and compliance with guidelines
  • Provide prompt feedback and manage audits for client files

ABOUT YOU

  • Experience in community/homebased care services
  • Strong customer service and communication skills
  • Proven leadership and conflict resolution abilities
  • Attention to detail and problemsolving skills
  • Qualification in Aged Care, Disability Services, Community Services, Nursing or Allied Health discipline preferred.
  • Previous Team Leader or people management experience
