The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
About the role
We are seeking a values-driven and proactive Case Manager You will be required to provide case management support in a community or residential based setting to assist housing stability, improve social inclusion and ensure service delivery is aligned with the national and state strategic plans within the National Homelessness Stream Model of Care.
Reporting to the Team Leader, this is a casual position, based in Spring Hill, QLD. Salary and conditions are in accordance with Social Community Home and Disability Services Crisis Award Level 2.
You will successfully
- Establish relationships with individuals that access the service that encourage self-determination and participation
- Provide case management support to individuals in accordance with TSA case management models and standards of practice
- Have the ability to complete thorough assessments, inclusive of needs, risks and vulnerabilities
- Conduct ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit
- Conduct individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual
- Implement case plans with active individual participation
- Display high levels of emotional intelligence, with an awareness of how personal values and beliefs can impact work
- Participate in regular supervision and case review meetings and team development activities
You will have
- Tertiary qualification in Community Services, Social Work or related field. Degree level qualification preferred
- Relevant experience in a social service environment including in client assessment and case management
- A current QLD Blue Card (Working with Children's Check)
- A QLD Driver's Licence
- A First Aid Certificate
- A national police record check is required
- The ability to work weekends and night shift as required
