Dementia Support Worker

**About the role**
A rewarding opportunity for a passionate Dementia professional to join the Dementia Support Australia (DSA) services team at HammondCare on a** Fixed Term Full-time contract until January 2024 **at our Horsley, Illawarra site with opportunities for future development.

The Dementia Support Worker will support the provision of behaviour management for people living with dementia as they transition from hospital to a chosen residential aged care home. As part of Dementia Support Australia’s new Acute to Residential Care Transition Service (ARCTS), the Dementia Support Worker will be a key role in the success of the program.

**Responsibilities**
- Work collaboratively with the person living with dementia, their carers, hospital and aged care staff, as well as any other health teams involved to understand the person living with dementia with best-practice support and care
- Support the transition from hospital to residential aged care whilst implementing recommendation plans and modelling the implementation of strategies to staff
- Ensure service delivery to clients operates in line with the underpinning principles of assessment and intervention within the psycho-social case management approach of the Dementia Centre.

**What do we need from you?**
- Demonstrated experience in dementia care and support within residential and community aged care settings, particularly in supporting personal care.
- Effective time management and ability to prioritise in a busy work environment
- A current Australian drivers licence
- Flexibility to travel intrastate and interstate on a regular basis to support the delivery of Dementia Centre services.
- Excellent problem solving and decision-making skills
- Ability to prioritise multiple tasks and meet deadlines
- Excellent written and verbal communication skills
- Empathy and patience
- Relevant qualifications in Allied Health, Enrolled Nurse (EN), Aged Care, Cert III or IV in Individual Support or similar highly desirable
