About the Advance Project


The Advance Project has recently launched a new suite of dementia-specific resources and training that aim to make initiating end-of-life conversations and assessing palliative care needs of people living with dementia easier, to enable better care.

The training is specifically designed for frontline staff working in aged care settings in Australia.


A new project funded by Aged Care Research & Industry Innovation Australia (ARIIA) will involve working with champion sites within HammondCare's residential aged care services to co-design implementation approaches, supporting staff to embed the resources in practice, and formal evaluation of the new resources.


About the role


The role of_ Clinical Support Lead, Advance Project Dementia _works within the Mission and Mission in Action of HammondCare alongside site-specific Working Group members, local residential aged care team members and the project team.


The _Clinical Support Lead_ will provide support for local frontline staff to undergo training and implement selected _Advance Project Dementia_ resources into routine care.

They will implement a co-designed local implementation plan and evaluation process, and contribute to design of broader implementation of the Advance Project Dementia resources and training across HammondCare, informed by the evaluation findings.


This role is offered on a max term contract working 2 days per week for 31 weeks. The role can be based at either Woy Woy, NSW or Horsley, NSW.

About you

  • Commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action.
  • Experience working as a Registered Nurse (RN5+) in aged care settings, including provision of advance care planning and palliative care for people living with dementia and their families.
  • A passion for improving advance care planning and the palliative care for people living with dementia and their families.
  • Clinical experience in facilitating advance care planning discussions and/or providing palliative care for aged care residents, including people living with dementia.
  • Coaching and mentoring experience with health professionals and a supportive communicator.
  • Demonstrated ability to effectively build and manage relationships with staff participants and relevant stakeholders.
  • Ability to work both independently and as part of a team.
  • Strong computer skills including Microsoft Office software.
  • Strong planning and organisational skills with the ability to prioritise tasks to meet deadlines.
  • Demonstrated experience in delivering education to aged care staff/health professionals.
  • Willing and able to work onsite at Woy Woy and/or Horsley location using own transportation. Travel to other locations may be required from time to time.
  • Capacity and willingness to support data collection for the evaluation, with support and supervision from the project team
  • Experience working clinically in HammondCare Residential Aged Care setting highly desirable
  • Postgraduate level qualification in relevant area desirable.
  • Project management experience desirable.
