**About the role**
A rewarding opportunity for a passionate Dementia professional to join the Dementia Support Australia (DSA) services team at HammondCare on a** Permanent Full-Time basis **at our Residential Facility at Horsley.

This role will ensure the delivery of the DSA service with assessments, case management, supporting, mentoring and modelling to reduce the impact of behaviours on people living with dementia, their carers and staff.

**Responsibilities**
- Provide dementia specific expert consulting and advisory services within and outside of HammondCare.
- Develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia
- Develop best practice support and care with the ability to develop and implement client-specific advice, recommendations and responding to changes in behaviour.
- Assess clients with behaviours related to dementia and provide resources and ideas for effective care provision and support for that person's needs.
- Access current evidence-based guidance and knowledge and ensure it is aligned with the Dementia Centre
- Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
- Work on other projects and assignments as required and in line with areas of skill, expertise and knowledge.
- Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures

**What do we need from you?**
- Expertise in supporting people living with dementia
- Experience in Australian residential aged care and or community aged care
- High level of negotiation, communication and relationship building skills
- Excellent problem solving and decision-making skills
- Ability to prioritise multiple tasks and meet deadlines
- Excellent written and verbal communication skills
- Excellent stakeholder management
- A current Australian drivers licence and willingness to travel
- Empathy and patience
- Relevant qualifications in Allied Health, Nursing, Aged Care or Mental Health highly desirable
