**Position Overview**:
We are seeking a dedicated Support Coordinator to join our team. As a Support Coordinator, you will play a crucial role in ensuring the seamless delivery of care and support services to our clients. You will work closely with families, caregivers, and our internal team to coordinate schedules, manage resources, and address the unique needs of each individual we serve.
**Key Responsibilities**:
- Coordinate and schedule support services for clients, ensuring timely delivery and adherence to care plans.
- Serve as the primary point of contact for families and caregivers, addressing inquiries and providing updates on service delivery.
- Collaborate with healthcare professionals and community resources to facilitate comprehensive care plans.
- Maintain accurate records of client progress
- Conduct regular assessments to evaluate the effectiveness of support plans and make necessary adjustments.
- Provide guidance and support to caregivers, ensuring they have the resources and information needed to deliver high-quality care.
**Qualifications**:
- Diploma in Community services, relevant qualifications in allied health, social work and disability care.
- Proven experience in care coordination, case management, or a related role.
- Strong organisational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and manage multiple priorities.
- Compassionate and empathetic approach to client care.
