**Key Responsibilities**:
- Conducted client needs assessments and developed and implemented individualized careplans.
- Coordinates medical, social, and psychological support services for seamless transition.
- Communicate regularly with clients and their families to provide ongoing support andfeedback.
- Maintain client files and records to ensure compliance with industry standards.
- Train and mentor team members to improve service quality.
- Build partnerships with community and healthcare organizations and participate inoutreach activities.
**Requirements**:
- Work experience in social work, nursing, or related field.
- Relevant work experience and knowledge of the senior care industry.
- Excellent communication and coordination skills, compassion, and patience.
- Ability to work independently and collaboratively within a team and adapt to a fast-pacedenvironment.
