**About the Role**:
The Assessment and Transition Coordinator undertakes prisoner risk assessments using the approved CV assessment instrument and provides a range of support and services to assist prisoners to transition into, through and out of prison.
This includes participating in the Case Management Review Meetings to develop and review progress against local plans; coordinating prisoner involvement in transitional programs; liaison and provision of assistance to external service providers; facilitating referrals and delivering and coordinating the delivery of the programs.
**About You**:
To be successful in this role, you will have:
- Demonstrated understanding of transition and reintegration issues and needs of prisoners.
- Demonstrated ability to administer risk assessment tools with individual prisoners.
- Demonstrated capacity or aptitude to work with clients who are difficult to engage individually and in group work settings.
- Capacity to work collaboratively within a prison setting.
