The model of care is:

- Carers are supported to access and navigate required health services
- Multidisciplinary assessment for children who are identified as having complex needs will be provided within the PTGH team through comprehensive multidisciplinary assessment and the development of a health management plan for each child
- Health management plans and treatment recommendations will be shared with child protection, who are responsible for following through on plans
