Are you passionate about making a positive impact in the lives of individuals with disabilities? Do you possess exceptional organizational and communication skills? We are currently seeking motivated individuals to join our team as an NDIS Case Manager and Support Coordinator Assistant.
As an NDIS Case Manager, your primary responsibilities will involve attending appointments for sign-ups, assisting clients when shifts need filling, and maintaining accurate records of clients' progress. You will play a vital role in the delivery of personalized care plans, ensuring clients receive the necessary support to achieve their goals and live fulfilling lives.
**Key Responsibilities**:
- Attend scheduled appointments to assist clients with NDIS sign up, onboarding and medical appointments.
- Collaborate with clients to identify strategies to achieve their NDIS goals
- Maintain up-to-date and accurate records of clients' progress, ensuring compliance with regulatory requirements.
- Liaise with community services and organizations to secure additional resources and support for clients.
- Work closely with clients, their families, and support networks to develop and review care plans tailored to individual needs.
As a Support Coordinator Assistant, you will be responsible for attending regular appointments and assisting in the management of leases to create effective care plans. Your role will involve coordinating support services for clients under the NDIS program.
**Key Responsibilities**:
- Attend scheduled appointments with clients and communicate weekly with each client.
- Collaborate with management and attend relevant stakeholders meetings.
- Assist in the development and implementation of comprehensive care plans tailored to individual needs.
- Maintain accurate records through our programs
- Support the Operations Manager in providing ongoing support to clients and their families.
Minimum working hours for this position is 16 hours per week on a part-time basis with the possibility of increasing hours based on performance and client needs.
To support you in your role, you will receive a company mobile and fuel allowance.
Key Requirements:
- Minimum 2 years experience within disability or nursing
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Knowledge and understanding of the NDIS program and its requirements
- Ability to problem-solve and work effectively in a fast-paced environment
- Valid driver's license and access to a reliable vehicle
- Valid Blue Card and Yellow Card
**Job Type**: Part-time
**Salary**: Up to $33.00 per hour
Expected hours: 16 - 20 per week
