At Support Well we promotes a high quality of life to clients. we are currently looking for a NDIS Support Coordinator who are passionate about delivering high-quality service.

As a Support Coordinator, you will assist with Ndis participants in existing support, building their capacity and achieving their Ndis goals. A typical tie in the life of Support Coordinator may involve setting up service agreements, Maintain Support plans, preparing Plan review, conducting risk assessment, care team meetings etc.

Someone who has an experience in case management in community services such as a disability mental health, caseworker or a knowledge of Ndis plans, NDIS funding and budgeting. Previous experienced in Support Coordinator is highly recommend.

The position is located in Melbourne and will require the person to travel to other locations with the region.

**Job Type**: Casual

Pay: From $40.00 per hour

Expected hours: No less than 5 per week
