Support Coordinator/recovery Coach

Cooma Challenge, established in 1954, is proud of its history in the Snowy-Monaro region supporting children, young people and adults with disabilities and, more recently, supporting older people. As we have grown over the years, we have maintained our overriding commitment to quality care and services, tailored to the needs of individuals, enabling them to live the life they choose.

We employ 80 people, including people with disability participating in supported employment, and we deliver services to more than 240 people in our local community.

We have a role in Support Co-Ordination/Recovery Coaching. The role is a NDIS funded role reporting directly to the CEO.

The role involves:

- Develop and co-design supports with clients and work collaboratively with them to make the most of their NDIS plan, accessing both formal and informal supports to build capacity.
- Supporting clients to implement and use their NDIS plan and assisting them to make informed choices about their supports to meet their individual needs and goals.
- Development, co-ordination, and evaluation of quality supports using supported decision-making strategies within a recovery framework.
- Developing individual Care Plans to support clients in their daily lives that foster maximum independence, quality of life and participation at home and in the community.
- Assist with plan reviews and documentation.
- Ensure that client service agreements and service bookings are completed.

**About you**:
You will have:

- Considerable experience in the disability, aged care, or health industries, including some experience in leadership or co-ordination.
- Formal qualifications in a human services field, and/or management/ leadership.
- High level communication and interpersonal skills, along with solid relationship-building skills to engage with a range of individuals including clients and their families, staff and other stakeholders and providers of supports.
- A strong commitment to the inclusion and empowerment of people with disabilities.
- Strong teamwork and self-management skills.
