- Provide case management outreach and support to people who are homeless or at risk of homelessness to access safe, affordable accommodation and prevent their return to the homelessness service system.
- The Salvation Army Homelessness Gippsland operates in a manner that is consistent with the aims, objectives and values of The Salvation Army and the guidelines provided by Department of Families, Fairness and Housing and Commonwealth Department of Social Services.
- Case Managers provide information, advice, advocacy, liaison and referrals to people who are assisted using a strength-based approach to identify goals to empower them to work towards establishing safe, secure, affordable housing. Additionally, case managers develop person centred and holistic case (and or safety) plans to ensure the individual needs and risks of each client is met throughout their service period with The Salvation Army Homelessness Gippsland.

- Role Responsibilities
- Strong networking relationships are formed and maintained with relevant stakeholders to ensure

coordination and responsiveness to meet the needs of homeless people.
- Effective relationships are established with clients that encourage self-determination and

participation.
- Case management support is provided to clients in accordance with TSA case management models and standards of practice.
- Individual case, safety and or private rental action plans are developed in collaboration with clients

and include a range of interventions that address the needs, risks, and goals of clients. All plans are

regularly updated and reviewed with the client at a minimum standard of every 3 months.
- Accurate, up to date client records and data is maintained for all clients in accordance with TSA

standards and procedures using TSA information systems. Case notes are to be completed within 24

hours of each client contact and documents uploaded to TSA information systems.
- Risks to clients, staff and TSA are identified and bought to the attention of Management as soon as

possible. Relevant documentation is completed in accordance with WHS policy.
- Any other tasks identified by the management team are to be completed within the required
- Qualifications and Experience
- Relevant tertiary qualification in social work, community services, youth work, psychology or community development - diploma level (minimum)
