The role you could play

We have an exciting opportunity for a Social Worker to be the fist point of contact with people attending the Access Health Services in St Kilda.

You will play a central function to the Access Health daily activities including: engaging service users, providing orientation, assessment, triage, information, referral and advocacy to people attending the service, managing the flow of service users through the program and coordinating activities and information to other Access Health professionals.

Responsibilities include
- Provide a safe and nurturing environment for all people attending services
- Work in a manner consistent with trauma informed practice, harm reduction principles and with a good understanding of the social model of health
- Conduct risk assessments and brief interventions
- Work collaboratively with a multidisciplinary team and provide administrative support to other health professionals
- Communicating effectively and professionally with internal and external stakeholders working towards the best possible outcomes for people using services
- Oversee and lead specific portfolios identified by need such as, co-facilitation of drop in groups

This is a permanent part time position (15.2 hours per week). Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award Level 5.
- Bachelors degree in Social Work, Social Services or similar
- Relevant experience in a social service environment preferably the health, AOD, homelessness and/or mental health sector
- A current VIC Drivers Licence
- Valid VIC Employee Working with Children Check
- Willingness to undergo a national police record and work eligibility check
