About the role
An exciting opportunity for a newly created Allied Health Interdisciplinary role to facilitate a coordinated care approach and support timely discharge planning of our medical patients.
This role will work closely with our geriatricians, nurse unit manager and allied health teams to complete a comprehensive assessment, provide an optimal discharge plan for patients from day of admission and reduce length of stay.
Key responsibilities:
- Undertake interdisciplinary assessment of medical patients at Cabrini.- Ensure relevant allied health disciplines are involved in patient care in a timely manner.- Work with the multi-disciplinary team and other key stakeholders to ensure high quality, patient-centred care and to optimise discharge planning.- Lead interdisciplinary service development, and undertake quality improvement and evaluation of service.
About you
You will also have:
- Post graduate qualification (Masters level) or working towards.- Extensive experience in assessment, treatment, and management of medical patients.- Able to lead change with established skills in organisation, planning and priority setting.- Demonstrated effective interpersonal, written and verbal communication skills.- Strong stakeholder (doctor, staff, patients, carers and family) engagement skills.- Demonstrated ability to perform effectively under pressure and to prioritise workloads.- Demonstrated understanding and commitment to the principles of continuous quality improvement.

Cabrini is a healthcare provider that is captured by the COVID-19 and seasonal influenza vaccination requirements released by the Victorian government. It is a condition of employment that all staff receive the required vaccines, unless they have a relevant medical exemption. Vaccination evidence or evidence of a medical contraindication must be provided prior to commencement.
