**Overview**

Are you passionate about improving healthcare delivery in our community? We are seeking a dedicated integrated Care Coordinator to join our team In this pivotal role, you will report to the Operations Manager of Rehabilitation and Aged Care, driving the effective implementation of our regional Out of Hospital Strategy.

Your mission will focus on enhancing patient care and community health outcomes by:

- Reducing emergency department presentations and hospital admissions
- Minimising the length of stay for patients in CHSA Hospitals
- Facilitating timely transfers of patients from metropolitan health services to their communities
- Promoting increased access to health services for vulnerable groups
- Coordinating the Rapid Intensive Brokerage Support (RIBS) Program in accordance with established guidelines

**Essential Criteria**

To excel in this role, you should possess a strong blend of qualifications, experience, and personal attributes that align with the goals of the Out of Hospital Strategy/Better Care in the Community Program:

- Strong interpersonal and communication skills
- Proven project management experience
- Leadership abilities and team collaboration
- Ability to evaluate outcomes and drive improvements
- Experience in rural/remote community settings
- Relevant qualifications in health or social care
- Knowledge of chronic disease management
