**Case Manager - Aged Care**
**Your Role**
As the Customer Support Manager each day will look a little different and your key responsibilities will include:
- Conducting reviews for our clients, building, and revising their care plans, goals, services and expenditure
- Establish Home Care Agreements and ensure all compliance and care governance documentation is completed accurately and correctly
- Developing and maintaining excellent relationships with clients, client’s family and advocates, staff, funding bodies, doctors and other allied healthcare professionals
- Assisting consumers to make purchases through their Home Care package which are in line with their goals and government regulations
- Making process improvement and implementing improvement requests as required
- Contributing to growth of the Home Care Package program
**What you’ll bring**
As a Customer Support Manager, you will bring with you a commitment to delivering exceptional service to our clients. Your experience in the Aged Care sector will enable you to:
- Provide advice and build care plans
- Manage funding requirements to ensure our clients can achieve the best care and support to brighten their lives.
- Deliver high quality, client centred care
To do well in this role you will have:
- Experience with care planning processes, including assessment and establishing client goals
- A customer focus with excellent communication and problem-solving skills
- Appropriate experience or qualifications in Nursing and /or, Community Services & Frontline Management
- Ability to manage time effectively in an environment of changing priorities
- Experience with Homecare & My Aged Care packages and funding requirements
