Hume City Council's Community Support Services include delivered meals, property maintenance, social support, community transport and senior citizen centres and we have an exciting opportunity for an **_experienced Team Leader _**to join our innovative and supportive team.

**What you'll be doing**:

- Lead and manage the day-to-day operations of Council's Community Support Services ensuring operations are consistent with Council and Government policies, Regulatory Service Standards and accepted professional best practice.
- Coordinate the provision and delivery of Community Support Services, ensuring individual client needs and expectations are met.
- Ensure Community Support Services are delivered by staff appropriate skills and experience to provide quality and responsive service that meet clients needs.
- Responsibility for coordinating client care and management, including managing referrals and waiting lists for all Community Support Services, ensuring appropriate allocation of services and matching to groups is undertaken and standardised priority of access is implemented as designed, and conducting reviews of Community Support Services clients as required.
- Provide relevant activity data, statistics =, performance reports and other information as required.

**The skills and experience you'll bring to the role**:

- An awareness and understanding of issues confronting older people with disabilities in a home and/or community setting.
- Knowledge of other relevant community support agencies and service providers
- Knowledge of My Aged care (MAC)Service provider portal and experience with client file management systems.
- Ability to supervise staff including an ability to implement staff training and development and ensure adherence to Equal Employment Opportunity and Occupational Health & Safety policies procedures.

**Required Certifications & Qualifications**:

- Tertiary qualification in Community Services, Allied Health, Human Services, or a related field, and experience in working with older people and people with disabilities in a home or community setting; or lesser formal qualifications with substantial experience in working with older people and people with disabilities in a home or community setting.
- Strong knowledge of Home and Community Care Program for Younger People (HACC-PYP) and Commonwealth home Support Programme (CHSP).
- Knowledge of My Aged Care (MAC) service provider portal and experience with client file management systems.
- Demonstrated ability to direct, supervise and support staff.
- Current driver's licence.
