**About our Transition Care Program**

The Transition Care Program is a major policy initiative between the Commonwealth Department of Social Services and the State Department of Health. TCP offer short term support and active case management for older people to enhance their quality of life. The program is goal orientated to optimise the client’s functional capacity and to finalise and access their longer term care arrangements or maintenance at home. The program provides a range of services such as case management and low intensity Allied Health therapy.

TCP aims to minimise inappropriate extended hospital lengths of stay and prevent unnecessary presentations or admission to the Emergency Department.

**About The Role**

The Administration Officer plays key role within the Transition Care Program, providing a customer and program focused service to clients, their families and staff members. Working in a busy team-based environment the Administration Officer provides efficient and effective administration support to the TCP staff. The Administration Officer contributes to optimum client care and actively contributes to the smooth and efficient functioning of the program. This role is an entry level specialist role.

- Demonstrated experience (aligned to position description noting no reference to years of experience)
- Current Australian drivers licenses
