To be considered for this rewarding role you will have:

- A tertiary qualification e.g.: Bachelor in Nursing, Allied Health or Social Work / Welfare or Human Services
- Essential to have previous experience working in the aged care and or the disability field, ideally as a Care Manager / Case Manager
- Current National Police Clearance (_within 6 months of check_), Working with Children/Blue Card and Disability Services Positive Exemption Notice Card (“NDIS” or Yellow Card) or a _willingness to obtain_
- A valid Driver’s License and reliable vehicle
- Previous experience in Home Care Packages or relevant programs is required
- Demonstrated experience in managing client budgets
- Experienced in assessment and record keeping via databases and or client management systems, ie: Carelink + experience would be advantageous
- Able to work autonomously and as a part of a team
- Able to manage conflicting priorities - excellent time management skills required
- Ability to work within Organisational Policies and Procedures and Legislative Requirements

BodeWell is an approved provider with the Department of Health and provides government-funded Home Care Packages, Private Aged Care and NDIS Services across Queensland and Victoria. We are looking for a special person to join our team coordinating client care and support across the region.

This role ensures that our clients are supported to live independently in their home by assisting with Consumer Directed Care; including assessment, goal planning, referral for appropriate services, ongoing monitoring and review, withinthe Home Care Package Operational Guidelines and their allocated Home Care Package budget.

A mobile phone and laptop will be provided for your use. A shared Home Care Pool Vehicle is available for use during work hours. Use of your own vehicle may also be required, you will be paid for your work-related km’s traveled.
