**The Role**

Home, Acute and Community is a clinical program across Alfred Health. The program plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, and minimise long-term care needs. The Home, Acute and Community program oversees a diverse group of services promoting effective seamless care, from acute care back into the community. These services are provided in people’s homes, community centres and in hospital when indicated.

In response to the overall pressure within the Health Care system and seeking opportunities to do things differently, Alfred Health is undertaking a series of reform work based around three areas: Recover, Reset and Reform (3Rs). The 3Rs project is broken down into a number of workstreams, one being Better at Home. The aim of the Better at Home workstream is to maximise uptake of Alfred Health services to deliver more healthcare within patient’s homes. Models of care will incorporate home based and or virtual care.

The Change Lead - 3R Better at Home Workstream will work with the Better at Home workstream project team and support the achievement of project delivery. The Change Lead will work closely with key stakeholders from across the program, organisation and South East Metro Health Service Partnership and will be a vital and important member of the project team

The successful applicant will be someone who has experience in delivery of services and or projects with stated performance objectives within a complex organisation or environment.
