**About the Role**

This is a Permanent, Full Time position with at 76 hours per fortnight within the Diabetes Allied Health Team.

You will work in conjunction with the Diabetes and Allied Health team, and be responsible for identifying and coordinating care and support for the most vulnerable patients within the Diabeties service at the Royal Childrems Hospital (RCH). Thsi role will be involved in identifying and addressing psychosocial concerns, determining appropriate plans of support, referral to community bases services, and liaison with key organisations such as statutory child protection services.

Classification for this position will range from Grade 2 Year 1 to Grade 2 Year 4 in relevant discipline.

**What you’ll achieve**:

- Providing evidence-based psychosocial support and referral for children, young people and their families diagnosed with diabetes of any type, including:

- Identify and address the psychosocial needs of children and young people identified by the multidisciplinary team as being vulnerable or at risk
- Identify and address the psychosocial needs of newly diagnosed patients and provision of support around coping with diagnosis
- Develop and implement and appropriate plan of support to address issues identified
- Determine appropriate internal or external supports and refer as required
- Provide short term support and care coordination for vulnerable children and young people patients until they are engaged with appropriate support services
- Ongoing liaison as required with identified internal and external supports
- Coordinate and participate in multidisciplinary review of children and young people identified as vulnerable or at risk
- Act as a primary point of contact between the diabetes service and key agencies such as DFFH Child Protection
- Advocate for the needs of vulnerable children, young people and their families
- Participate in medical and joint nursing and allied health clinics
- Maintain clinical documentation, records, and data as per discipline specific guidelines and RCH procedures
- Provide clinical handover to required standard
- Provide child centred, family focused care through partnership with parents/ families

**Your skills and experience**:

- Current registration as an Allied Health Practitioner registered with the Australian Health Practitioner Regulation Agency (APHRA) or eligible for membership the relevant Allied Health professional body.
- Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio (PPP)
- Demonstrated capacity and skill in coordinating care for chronically ill patients and/or vulnerable children
- Demonstrated capacity to work constructively and collaboratively with a range of disciplines towards common goals
- Proven ability and skill to build collaborative relationships and work effectively within multidisciplinary teams
- For Allied Health applicants only: Entry to practice tertiary qualification and post qualifying clinical experience

**About the Diabeties service**

The Diabetes service at RCH cares for over 1500 children and adolescents with diabetes from metropolitan Melbourne and a further 300 children from regional Victoria with 2-5 new patients diagnosed per week. The RCH diabetes service is involved in active primary care, education, family support, research and advocacy for children and adolescents with diabetes. The Diabetes Allied Health Team is a key part of this service providing in-patient and ambulatory stabilisation of newly diagnosed patients, general inpatient and outpatient services and multidisciplinary outpatient clinics
