As the Carer Services Coordinator, you will be responsible for liaising with providers and carers to arrange respite services. This role will also support carers through the Carer Support Planning Process including an intake, registration and needs assessment, support planning, coordination, support and mentoring to ensure carers can be holistically supported by understanding their needs and appropriately referring them for supports.

This is a contract position until June 2024.

After hours on-call availability will be required overnight and on weekends, as needed and on a rotational basis, to arrange emergency respite for carers.
- Tertiary qualification in Social Work or other relevant discipline and/or ability to show demonstrated work experience relevant to the role
- Demonstrated experience in service delivery and telephone support work
- Knowledge of the community care and health systems and other programs relevant to carers
- Highly developed interpersonal skills, including the ability to effectively negotiate and liaise with a wide cross section of people, including internal staff, partners, service providers and the wider community
- Understanding of intake, assessment and referral processes provided as part of a telephone information, support and planning process
- Strong understanding of Mandatory reporting and privacy requirements
- Strong computer, administration and organisational skills.

Salary and Benefits

The Salary is around $93K+ super, pro rata for part time hours. Additional benefits include salary sacrificing benefits, flexible work arrangements, a highly supportive team environment, excellent training and development opportunities and health and wellness programs.
