Social Care Coordinator

Refapp (Testing) is looking for a passionate and dedicated Social Care Coordinator to join our team. As a Social Care Coordinator, you will be responsible for coordinating and implementing social care programs and services for our clients. You will work closely with clients, their families, and other healthcare professionals to ensure the highest level of care and support.
In this role, you will assess client needs, develop care plans, and monitor the progress of clients throughout their care journey. You will also be responsible for coordinating with external healthcare providers and community organizations to ensure seamless care transitions and access to necessary resources.
**Requirements**:
- Bachelor's degree in social work, psychology, or a related field
- Previous experience in social care coordination or a similar role
- Knowledge of social care systems, resources, and regulations
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work effectively with clients, families, and healthcare professionals
- Proficiency in computer systems and software
- Valid driver's license and access to a reliable vehicle
