About the role
- The purpose of the role is to support the implementation of best practice for patients who are identified with a diagnosis of chronic heart failure and are at risk of rehospitalisation, by: providing high level strategic advice and management; supporting research and quality improvement activities; providing clinical education courses related to heart failure accessible across Queensland; reviewing performance of teams; and engaging consumer participation in planning.
- The position coordinates heart failure services of nurses and allied health in 21 sites across 12 Hospital and Health services across Queensland to ensure high standards of practice and further service development.
- This role forms collaborative, strategic relationships with key stakeholders within 12 HHS and external partners

About you
**Desirable criteria**:
Clinical experience as a health professional
- An understanding of research and quality methods gained through post graduate study
- Experience in teaching and training and curriculum development

Essential
- Expert knowledge, or demonstrated ability to acquire knowledge, of heart failure management issues and priorities at a national and state level.
- High level knowledge of health service priority areas; the health needs of key population groups; and key health system issues (particularly related to cardiac care and chronic disease).
- High level ability to source, critically analyse and interpret research and information to underpin strategy and service planning.
- Ability to critically analyse numerical data, clinical evidence, and health service provision information in the Queensland context to assist to identify future priorities and issues to inform planning.
- Experience in managing projects in a large, complex service delivery organisation.
- High level written communication skills, for a range of audiences including ability to compile reports and submissions.
- Excellent and demonstrated interpersonal skills in negotiation, influencing, advocacy, communication, consultation and harnessing the collective efforts of diverse stakeholders at strategic and operational levels, including health professionals, government and non-government stakeholders.
