We offer a range of services including Harm Reduction and Primary Health Services as well as a range of treatment types (including residential treatment centres, community-based support programs, counselling, and withdrawal management services).

About the role

We are seeking a values-driven and proactive Social Practitioner to facilitate the Therapeutic Day Rehabilitation (TDR) program utilising a combination of strategies including group work, one to one intervention and family support for individuals experiencing Alcohol and Other Drug (AOD) related problems.

Reporting to the Senior Practitioner this is is a permanent, full-time position based in Bendigo,VIC. Salary and conditions are in accordance with the SCHADS Award Level 5.

You will successfully- Screen, assess and match referred clients to the most appropriate treatment service after liaison with catchment-based intake, allied health and other health care professionals.- Establish effective relationships with clients that encourage self-determination and participation.- Provide case management support to clients and their families.- Provide ongoing assessment of the client’s issues, needs and circumstances from initial intake to exit.- Deliver therapeutic group sessions in accordance with the client needs.- Maintain accurate, up to date client records, including group activities and data.- Identify any risks to clients, their families, staff and TSA and bring them to the attention of the Team Leader as soon as possible.- Identify service gaps and make recommendations for service delivery improvements to the Team Leader.

You will have- A current and valid VIC Working with Children Check.- A current VIC Driver's licence.- Bachelor of Human Services, Master of Social Work Diploma in Health or Community Services.- AOD skill set or willing to obtain within 12 months.- Relevant experience in a social service environment.
