We offer a range of services including Harm Reduction and Primary Health Services as well as a range of treatment types (including residential treatment centres, community-based support programs, counselling, and withdrawal management services).

About the role

We are seeking a values-driven and proactive Overdose Prevention Practitioner to provide support to clients who are at high risk of overdose through assertive Care & Recovery Coordination/Case Management

Reporting to the Manager of Intensive Services this is is a permanent, full-time position based in Bendigo,VIC. Salary and conditions are in accordance with the SCHADS Award Level 5.

You will successfully- Conduct initial assessment activities including competently assessing client risk and need.- Liaise with allied health and other health care professionals to obtain the necessary medical and other healthcare records to assist referral pathways.- Develop an effective relationship with the client to assist them to deal with issues related to their A&OD misuse.- Develop client care plans with clear strategies which monitor and measure outcomes and desired goals whilst initiating client ownership.- Deliver evidence based interventions in line with client centered treatment that aligns with AOD training and qualifications according to complexity of the client and family needs within a recovery framework.- Provide collaborative overdose response plans and education to each client.- Provide case management support to clients.- Maintain accurate, up to date client records and data for all clients in accordance with TSA standards and procedures using TSA information systems.- Respond to referrals in a flexible and timely manner.- Identify any risks to clients, their families, staff and TSA and bring them to the attention of the Team Leader as soon as possible.- Identify service gaps and make recommendations for service delivery improvements to the Team Leader.- Provide assistance with the service delivery of the Therapeutic Day rehab as requested by the Team Leader.

You will have- A current and valid VIC Working with Children Check.- A current VIC Driver's licence.- Minimum bachelor’s degree level qualifications in human services, social work, community services, or relevant area of study.- Relevant experience in a similar role, preferably within the alcohol and other drug sector.- AOD Skill set or willingness to undertake within the first 12 months of commencing employment.
