An exciting opportunity has arisen to join their team in the position of NDIS Support Coordinator.
**The Role**
Your role will focus on coordinating support for NDIS clients, and delivering strategies, guidance and support to enhance their mobility, independence and wellbeing. This will include:
- Coordinating the safe delivery of quality holistic care to the client and monitoring client progress
- Determining health and wellbeing maintenance and promote strategies aimed at encouraging independence and assisting the client to maintain a quality of life
- Engaging with allied health professionals and community supports to enhance client care delivery
- Providing supports and services in alignment with client budgets and available funding
- Coordinating comprehensive assessment of each client in collaboration with the client and their family or carer
- Coordinating services in line with the funding bodies' requirements, Social Justice and Access & Equity principles, WHS requirements and the organisation’s philosophy, mission, policies and procedures
- Developing associated documents and reporting, as required
**What You'll Need**
In order to succeed in this role, you will require the following:
- 1+ years' experience as an NDIS Support Coordinator
- Demonstrated understanding of the needs and issues of people living with disabilities and their carers
- Demonstrated experience in needs assessment and management of NDIS participants
- Understanding of NDIS funding instruments with experience in managing client budgets
- Excellent written and verbal communication skills
- Computer literacy
- Valid National Police Check (or willingness to obtain)
- Valid NSW Driver's Licence and own vehicle
Although not essential, relevant qualifications would be advantageous
