**Grade 3 Allied Health Professional**

Description of the Role / Your New Role- Support safe and timely discharge for NDIS participants to return to the community.
- Provide excellent evidence based and patient centred care including discharge planning, care coordination, and client advocacy
- Utilise specialised skills and knowledge in relation to NDIS, communicating between patients and caregivers, and supporting ongoing community needs
- Demonstrate excellent communication strategies with patients, relatives, staff and relevant services in order to provide appropriate services, information and leadership
- Actively lead and participate in Allied Health quality improvement initiatives and research

Description of the Person / About You- You have demonstrated health industry experience with a collaborative leadership style
- You have excellent communication skills and a passion for continued professional development for yourself and others.
- You are a Qualified health professional with current AHPRA registration (where relevant), or eligible for membership of relevant professional association
- You value working with supportive and dedicated health professionals and multidisciplinary teams.
