Community Health Nurse (Grade 4), Barwon MDC – Geelong West. This is a permanent part-time role (32 hours per fortnight) within Barwon Health, partnering with the Barwon Multidisciplinary Centre to provide holistic health care to victims/survivors of sexual assault and/or family violence and their families.
Responsibilities- Advise victim/survivors on the health implications of sexual assault
- Perform health assessments and facilitate access to other health services as appropriate
- Provide individual health and wellbeing support, advocacy, and nurse-specific counselling; educate stakeholders about health issues affecting MDC clients
- Establish effective local health care networks to improve care and contribute to the state-wide MDC nursing network
- Work within the MDC CHN team to triage and prioritise referrals

About The FacilityMajor sites include University Hospital Geelong, McKellar Centre, Barwon Health North, and residential aged care facilities. Mental health, community health and dental services are delivered across the Barwon South West Region, including Torquay and Colac.
About The RoleBarwon Health is recruiting a Community Health Nurse to enable access to holistic health care for victims/survivors of sexual assault and/or family violence. Working from the Barwon Multidisciplinary Centre (MDC), you will collaborate with Sexual Assault & Family Violence Centre counsellor advocates and case managers, Victoria Police, and Child Protection to improve health care for vulnerable individuals.
The MDC CHN service provides in-reach and outreach nursing from each MDC, operating from a trauma-informed approach. You will undertake initial needs identification, conduct holistic health assessments, develop and implement client-centered care plans, provide health education, increase health literacy, and assist clients to engage with health care services.
Key duties include health assessment, care coordination, education, advocacy, and establishing local networks to improve care for victims/survivors of sexual assault and family violence.
