Occupational Therapist (Community-Based) – Hybrid Role

Tamworth, Maitland, Newcastle & Surrounds

Join a team that's reshaping the future of community healthcare. EmpoWRreach, a specialist-led division of WoundRescue, is seeking a qualified and compassionate Occupational Therapist to support our growing caseload across aged care, NDIS, and private clients in the community.

This role offers the best of both worlds, in-home visits and supported work-from-home flexibility, withno set roster. You'll have full autonomy to manage your own schedule, allowing for genuine work-life balance.

Responsibilities

  • ConductingFunctional Capacity Assessments (FCAs)
  • CompletingHome Modification Assessmentsand prescribing assistive technologies
  • Driver OT Assessments(highly desirable)
  • Supporting planning, equipment recommendations, and ADL-based interventions
  • Working alongside our wound care specialists and generalist nurses to align therapy with holistic clinical goals
  • Preparing timely, professional reports that support funding applications and care plans
  • Maintaining clinical documentation within our digital case management systems
  • Building strong partnerships with clients, families, and referrers

Qualifications

  • Degree in Occupational Therapy and current AHPRA registration
  • At least 1 year of post-graduate clinical experience, ideally within community, aged care, or disability services
  • Experience in functional and home modification assessments essential
  • Driver OT assessment capability highly desirable
  • Valid driver's licence and access to a reliable vehicle
  • Strong written and verbal communication skills
  • Confidence with digital systems and remote work
  • Passionate about person-centred care and improving client quality of life
