We are seeking an experienced Registered Nurse (RN) to join our team as a Care Manager at our Home care in Perth South, western Australia. This full-time role will involve leading a team of support workers and overseeing the delivery of exceptional care to our valued Clients

What You'll Be Doing

• Supervising and managing a team of care staff to ensure high-quality, client-Centered care is provided
• Developing and implementing individual care plans to meet the unique needs of each client
• Monitoring the health and wellbeing of clients and providing appropriate nursing interventions
• Liaising with clients, families, and other healthcare professionals to coordinate care
• Ensuring compliance with all relevant aged care regulations and standards
• Participating in the continuous improvement of care practices and policies
• Providing mentorship and training to care staff
  
  

What We're Looking For

• Current registration as an RN with the Nursing and Midwifery Board of Australia
• Minimum of 3 years' experience in a similar aged care or nursing and home care experience role
• Demonstrated experience in leading and motivating a team of care staff
• Strong clinical skills and knowledge of aged care best practices
• Excellent communication and interpersonal skills
• Ability to work collaboratively with clients, families, and other healthcare providers
• Experienced in wound care management.
• trained in female and male catheters.
• Experienced in wound care management.
• Able to use critical thinking skills.
• Proficient in using technology and electronic medical records experience in conducting care plaining and assessment.
