
Outreach Navigator Specialist

A function of the Coordinated Intake (CI) program is designed to provide housing stabilization assistance and services to no-to-low-barrier, newly homeless sheltered homeless individuals and families who are housing ready. Under the CI Housing Navigation Department, directly supervised by Coordinated Intake Supervisors and supported by the Director of Emergency Housing Services, the Outreach Navigator will engage with unsheltered individuals, provide support, and facilitate transitions to stable housing. This position requires daily engagement with individuals experiencing homelessness, collaboration with various teams, and accurate documentation and supportive services for housing stability.
The Outreach Navigator will work collaboratively with enrolled participants under the Home for Every Neighbor initiative led by the City of Cleveland Project Management Team. The Outreach Navigator will assist participants with exploring rapid exit strategies from encampments into permanent housing, maintaining a caseload ratio of 1:20. The Outreach Navigator will assist participants with determining housing eligibility, program enrollment and housing coordination through public resources provided through the Home for Every Neighbor initiative. This position is temporary, concluding December 31, 2025.
To best serve program participants, the Outreach Navigator must advocate that individual choice is upheld during participation in the housing process. The Outreach Navigator is a highly collaborative role requiring teamwork and coordination amongst established community partners, such as Outreach, Housing Case Management and Unit Acquisition teams.
This is a non-exempt, union position, paid at $20.00 per hour.
Responsibilities
- Engage daily with individuals experiencing homelessness, including unsheltered participants.
- Collaborate with CI teams and community partners to document services for housing stability.
- Maintain and manage a caseload ratio of 1:20.
- Assist participants with exploring rapid housing exits from encampments into permanent housing.
- Determine housing eligibility, assist with program enrollment, and coordinate housing options using public resources.
- Advocate for participant choice throughout the housing process.
Qualifications
- Bachelor's degree in social, behavioral, or human services.
- Personal experience with homelessness or housing instability is highly valued.
- At least 2 years of professional experience in case management, community outreach, and/or the social service field.
