Part time role is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.

Within the Transitional Aged Care Program, we are proud of our energetic team that values kindness, collaboration, and compassionate patient care. You will play a pivotal role in delivering high-quality Occupational Therapy services to clients within both our community-based (Transpac) and residential (Transitional Care Unit) programs.

About the Role

As an Occupational Therapist within the Transitional Aged Care Program, you will deliver client-centred care that supports older people in achieving their goals for recovery, independence, and quality of life. Working closely with clients, their families and carers, as well as the wider multidisciplinary team, you will provide clinical assessment, intervention, and professional advice in line with best practice and departmental guidelines.

In this role, you will:

  • Provide evidence-based care and exercise sound professional judgment in clinical decision-making, with support from senior clinicians as required.

  • Support mobility, falls prevention, pain management, and functional recovery for clients in both community and residential settings.

  • Actively participate in ongoing professional development, supervision, and quality improvement initiatives to enhance client outcomes and service delivery.

  • Contribute to the professional network of your discipline, while engaging in opportunities for education, training, and clinical research activities.

  • Promote collaboration within the multidisciplinary team and liaise with CCLHD and government agencies to ensure clients receive comprehensive, coordinated care.

About You

  • Degree or Diploma in Occupational Therapy
  • Current Occupational Therapy registration with AHPRA
  • Demonstrated understanding of aged care needs, including mobility support, falls prevention strategies, pain management, and functional recovery.
  • Ability to work across sites within the CCLHD
