About The OrganisationThe Derbarl Yerrigan Health Service Aboriginal Corporation (DYHS), established in 1974, is the largest Aboriginal community-controlled health service provider in Western Australia. It provides care to over 20,000 Aboriginal patients across clinics in East Perth, Mirrabooka, Maddington, and Midland. Our staff deliver culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care, and care coordination including chronic disease management.

The NDIS Support Worker supports Aboriginal people with disabilities to access and navigate the NDIS. The role involves engaging with NDIA Partners, NDIS Planners, and Support Coordinators to provide culturally safe linkages to services. This is a Maximum Term Full-time contract ending on 31 May 2026, with the possibility of extension.
Key Responsibilities- Provide outreach to Aboriginal patients, building trust and rapport to facilitate NDIS access.
- Link potential Participants to community resources and support services.
- Assist participants with appointments, assessments, and documentation for NDIS access.
- Support participants in obtaining updates and completing forms.
- Refer ineligible patients to mainstream supports and community services.
- Support handover processes between participants, PITC, and NDIA, fostering positive relationships.
- Collaborate with community engagement activities and support networks.
- Help participants understand and manage their NDIS plans and processes.
- Assist with complaints, incident processes, and plan reviews as needed.
- Maintain patient records and provide regular reporting.
- Attend relevant training and stakeholder meetings.
- Perform other duties aligned with the role's objectives.
- Certificate IV or extensive experience in disability/NDIS sector.
- Excellent communication and interpersonal skills, especially with vulnerable populations.
- Knowledge of the NDIS scheme.
- High confidentiality, tact, compassion, and empathy.
- Strong computer skills and reporting ability.
- Ability to work independently and prioritize tasks.
- Valid WA Driver's license.
- Satisfactory Police Clearance and Working with Children check.
- Full COVID-19 vaccination.
- Aboriginal and/or Torres Strait Islander descent.
