About the Role:

As a Care Coordinator, you will be the key link between our clients, their families, and our care team. You will ensure that our clients receive the highest quality home care services by managing schedules, coordinating care plans, and providing compassionate support.

Key Responsibilities:

  1. Match clients with caregivers based on needs and preferences
  2. Schedule and coordinate in-home care services
  3. Communicate with families, caregivers, and healthcare professionals
  4. Monitor and update client care plans
  5. Ensure compliance with NDIS standards and aged care best practices
  6. Provide exceptional customer service and client support

What We're Looking For:

  1. 2 years Experience in aged care/disability care and knowledge of NDIS standards and Aged Care Standards preferred
  2. Strong organizational and communication skills
  3. Ability to multitask and work in a fast-paced environment
  4. A caring and compassionate attitude
  5. Proficiency with scheduling software and basic computer skills

Certificate III/IV in Individual Support (Ageing and Disability) or equivalent
