Overview

Care Team Leader – Ongoing Permanent Full-Time opportunity in the Brisbane Home Care team. The Care Team Manager leads the team of Community Care staff who provide direct care to clients. Your day-to-day involves guiding the team to deliver person-centred care within budget and service expectations.

Responsibilities
  • Ensure staff have a good understanding of individual client needs and preferences and can provide services tailored to each client.
  • Lead, support, and mentor the team; collaborate with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager as required.
  • Manage recruitment of new Care Workers and ensure appropriate validations, education, development plans, and resources are available.
Qualifications and Requirements
  • Certificate IV in Aged Care (Desirable) or HR or similar degree; minimum of 2 years relevant industry experience.
  • Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards, and the Commonwealth Home Support Programme.
  • Performance coaching experience and management of a team in a multi-site environment.
  • A current driver's licence, safe driving record, and willingness to travel between HammondCare's clients, sites, and services.
  • Strong organisational, administrative, and computer skills.
  • Empathy and patience.
