Service Coordinator CHSP at Care Connect - Community Care Provider – Join to apply for this role. Care Connect is a not-for-profit, approved Home Care Package provider that understands how important it is to remain independent at home. Office location: Bella Vista, NSW. The role supports hours across NSW and VIC.
Responsibilities- Serve as the primary point of contact for clients, assisting them in accessing services and navigating their care options.
- Promote the Care Connect proposition and maximise client outcomes by coordinating between clients, client advisors, and partnered service providers.
- Maintain a high standard of customer service, ensuring all interactions are handled with care and professionalism.
- Build strong relationships with clients, understanding their needs and providing tailored solutions.
- Collaborate closely with care managers and service providers to ensure the best possible client outcomes.
- Qualification or formalised training in Customer Service, Business Administration or equivalent.
- In the absence of a completed qualification, a minimum of 3 years' experience in a high-volume inbound phone contact role or related field with a proven track record of exceeding client expectations.
- Experience in CHSP or health/community services with care coordination is beneficial but not essential.
- Strong communication and time management skills; comfortable using Microsoft Office.
- Full Australian working rights; Current Police & Working With Children Checks; police checks can be arranged for successful candidates. Psychometric assessment is part of the selection process.
- Qualification or formalised training in Customer Service, Business Administration or equivalent.
- In the absence of a completed qualification, a minimum of 3 years' experience in a high-volume inbound phone contact role or related field with a proven track record of exceeding client expectations.
