Overview

Service Coordinator CHSP at Care Connect - Community Care Provider – Join to apply for this role. Care Connect is a not-for-profit, approved Home Care Package provider that understands how important it is to remain independent at home. Our collaborative, authentic and talented team enhance people's quality of life each and every day. This is a full-time, max-term position until 30/06/2026.

Office location: Bella Vista, NSW. The role supports hours across NSW and VIC.

Responsibilities
  • Serve as the primary point of contact for clients, assisting them in accessing services and navigating their care options.
  • Promote the Care Connect proposition and maximise client outcomes by coordinating between clients, client advisors, and partnered service providers.
  • Maintain a high standard of customer service, ensuring all interactions are handled with care and professionalism.
  • Build strong relationships with clients, understanding their needs and providing tailored solutions.
  • Collaborate closely with care managers and service providers to ensure the best possible client outcomes.
Qualifications
  • Qualification or formalised training in Customer Service, Business Administration or equivalent.
  • In the absence of a completed qualification, a minimum of 3 years' experience in a high-volume inbound phone contact role or related field with a proven track record of exceeding client expectations.
  • Experience in CHSP or health/community services with care coordination is beneficial but not essential.
  • Strong communication and time management skills; comfortable using Microsoft Office.
  • Full Australian working rights; Current Police & Working With Children Checks; police checks can be arranged for successful candidates. Psychometric assessment is part of the selection process.
Minimum Qualifications
