We are looking for a reliable and organised Case Manager to work 3 days per week. This role combines administrative tasks with direct participant contact, ensuring files are up to date and wellness checks are completed regularly.
Key responsibilities:
• Maintain and update participant files and records
• Complete general administration and prepare reports
• Conduct regular participant wellness checks (in person and over the phone)
• Liaise with participants, families, and support staff
• Report any concerns to management promptly
What we're looking for:
• Experience in case management, support coordination, or a similar role (NDIS experience preferred)
• Strong organisational and communication skills
• Ability to work independently and follow direction
• A genuine interest in participant wellbeing
Details:
- 3 days a week
- Reports to managment
Pay: Competitive market rate – $33 to $40 per hour (based on experience)
