The Home Care Services Manager is responsible for the effective management, development and expansion of Home Care services at Rossmoyne Waters on behalf of Adventist Care. This role aligns with Adventist Care's vision and mission, focusing on business development and quality management activities in accordance with the strategic plan for expanding Adventist Care's Home Care services within our communities.All tasks must be performed within the WHS standards

Key Responsibilities

Organisation / Teamwork:

·Oversee the successful development, management, and quality evaluation of Home Care services on the Adventist Care business site.

·Collaborate and contribute to the ongoing strategic development and business innovation strategies of Adventist Care as a member of the organisational senior management team.

·Communicate effectively and responsively with all stakeholders within the Adventist Care organisation and the wider community, in line with corporate objectives and goals.

·Provide regular Home Care Program reports monthly and as required by the Chief Executive Officer, and Board of Directors of Adventist Care.

·Liaise and consult regularly with the Chief Executive Officer and the Home Care Management Committee, to ensure effective communication and teamwork regarding all matters of Home Care service provision to customers and the community of Adventist Care.

·Manage the ongoing professional development needs of all employees to ensure strong team performance and alignment with organisational objectives for Home Care service delivery and continuous quality improvement.

·Chair Home Care Program meetings, collate and distribute meeting minutes to all relevant persons/attendees.

Customers / Stakeholders:

  • Drive the growth of Home Care services, ensuring alignment with Adventist Care's strategic objectives.
  • Actively promote the Home Care Program to independent living customers and their families at Adventist Care, with the intent to facilitate customer referral and access, and continuously grow the number of customers requesting Adventist Care as their preferred service provider for their Home Care service needs.
  • Embed continuous quality improvement practices and processes into the Home Care Program, ensuring the maintenance of quality and consumer satisfaction in service delivery, consistently meeting their needs and expectations.
  • Collaborate with all Home Care Program employees to foster a service culture of responsiveness and flexibility in meeting consumer needs and expectations within the Consumer Directed Care model, while ensuring service delivery aligns with organisational objectives relevant to the effective and efficient use of all financial, human, and physical resources allocated to Home Care.
  • Ensure every Adventist Care Home Care client has an executed Home Care agreement prior to the commencement of services.
  • Establish a robust process that provides timely reviews and amendments to Home Care agreements when Adventist Care Home Care customers request changes to services, and ensure such changes are communicated to the Adventist Care accounts department.
  • Oversee and manage effective and collaborative working relationships with internal and external Home Care workers, key personnel, and other service providers, including assessment and troubleshooting interventions.
  • Facilitate positive organisational working relationships with other external Consumer Directed Care stakeholders and service providers, facilitating consumer referral and access in circumstances where consumers relocate or elect to change their service provider
  • Manage and evaluate the delivery of Home Care services within the Home Care Program of Adventist Care, ensuring compliance with government legislation and industry standards relevant to a Consumer Directed Care (CDC) model.

Quality, Risk and Compliance:

  • Stay up to date with Aged Care / Home Care legislative changes and consult with the Chief Executive Officer on amendments requiring integration into Adventist Care systems and processes.
  • Lead by example in all quality activities within the Adventist Care quality framework.
  • Actively promote and engage in customer access rights to Adventist Care quality improvement feedback systems, assisting customers with expressing their suggestions, comments, or complaints.
  • Manage the delivery of quality Home Care services for customers that are consistently safe and demonstrate best practice alignment with workplace health and safety standards in the immediate and surrounding workplace environment of Adventist Care.
  • Actively engage in the development and review of policies, related documents, and systems pertinent to Home Care in consultation with the Chief Executive Officer, ensuring compliance with Aged Care National Quality Standards for Home Care.
  • Conduct audits of clinical, compliance, and financial key performance areas as per agreed timeframes established in consultation with the Executive Care Manager and Chief Executive Officer, producing an action plan to resolve gaps in Home Care systems, processes, people, and culture in a timely manner and in line with Adventist Care's Vision and Mission statement.
  • Regularly liaise and consult with the WHS Risk Management Officer to facilitate best practice in the delivery of workplace health and safety standards for staff and customers across the site, including supporting injured workers to return to the workplace as soon as possible.
  • Manage the Home Care commitment to continuous quality improvement, including preparation of self-assessment, application for commencing services (as required), re-accreditation for existing business, and active engagement leading to successful achievement and maintenance of accreditation status for service provision in the Home Care Program.

Qualifications Required:

·Diploma in Community Care Coordination or Diploma in Business Management or Diploma in Applied Science (Nursing) or Bachelor of Nursing with recent experience (min. 3 years) in Community Care or Aged Care business administration/management or equivalent.

·Current National Police Check clearance.

·Current Driver's License – no restrictions; and

·Management experience working within Residential aged care, Community care and or Home Care sectors – minimum 3 years; and

·Proficiency in Microsoft programs such as Outlook, Word, Excel, Power Point and in addition familiar with electronic information systems such as e-tools, Sarah or equivalent, online claiming portals; and

·Proficiency in people and culture activities including recruitment, selection, retention, release, rostering.

·Prior experience in project management; and

·Excellent communication and presentation skills.
