We're looking for a confident and compassionate Home Care Manager to take the lead of our Home Care program. You'll be responsible for both the smooth day-to-day running of the service and its longer-term strategic direction, making sure we're not only delivering high-quality care, but also growing in the right way for the future.

From ensuring compliance with aged care standards, to strengthening systems and improving the client experience, to building partnerships that support sustainable growth, then this is a role where your leadership can make a lasting impact.

You'll bring a strong background in aged care or community services, paired with a genuine passion for helping people. You're calm under pressure, confident in your decisions, and the kind of leader who supports their team to do their best.

You're naturally curious and solutions-focused, and you always keep the client at the centre. Whether you're sorting out a service delivery issue, mentoring a team member, or mapping out the next phase of the program, you lead with empathy, integrity, and a clear focus on quality.

What your role looks like:

Oversee client services, staff rostering, and workforce management to deliver smooth, high-quality care

Ensure full compliance with Home Care Package requirements and proactively prepare for sector reforms

Implement effective systems and processes that enhance team performance and service delivery

Guide care planning that thoughtfully balances client needs with budgets and organisational goals

Drive strategic initiatives to grow and strengthen the program's impact

Cultivate partnerships that bring innovation and new opportunities to the service

You'll be a great fit if you:

Bring proven experience managing home care programs and a thorough knowledge of the Home Care Package framework

Are skilled at leading diverse, multidisciplinary teams in regulated, client-focused settings

Have a strong grasp of aged care legislation, compliance, and quality standards

Communicate clearly and build genuine, trusted relationships across all levels

Lead with calm confidence, empathy, and a dedication to delivering excellent care

Proven experience leading aged care, home care, or community service delivery programs

A strong track record managing operations across multidisciplinary teams

In-depth knowledge of Support at Home or similar aged care programs, with a focus on compliance and sector reform

Excellent communication and stakeholder skills—you build strong relationships at every level

A calm, steady leadership style and the ability to motivate and guide your team with purpose

About Us

For 60 years, St Bart's has supported people experiencing homelessness and severe and persistent mental health issues as they rebuild their lives. We started as a small homeless shelter for men and have grown into a progressive and inclusive organisation providing support to hundreds of Western Australian men, women and families across four key service areas. Our goal is to provide safe, supported accommodation and services to people who need it the most, regardless of background, age, gender or ethnicity.
