Function Nursing Facility Position Care Coordinator Introduction

A vacancy exists for a Renal Care Coordinator based at Life Renal Dialysis Newcastle / Vryheid, reporting to the Quality Auditor. The successful candidate will be responsible for facilitating the enhancement of more holistic disease management for patients by seamlessly integrating currently disjointed aspects of the renal journey. The role aims to improve patient care by supporting the Renal Unit Manager and the Multidisciplinary Team in care activities.

Critical Outputs

  • Financial Risk Management: Identifying deviations from the clinical pathway, managing financial risks related to clinical assessments, authorisations, re-authorisations (including clinical coding). Supporting the Bill Auditor to resolve funding disputes related to clinical pathway products. Monitoring and reporting on event costs, analyzing trends to identify deviations, and influencing responsible parties through presentations and discussions.
  • Clinical Risk Management: Conducting clinical pre-assessment following initial diagnosis. Acting as the primary contact for patients from post-diagnosis until they are equipped to self-direct care. Supporting patients and families with education about their care needs as they progress through the system.
  • Coordination / Facilitation Role: Building awareness of the Life Care Coordinator role among internal and external providers. Responding to criteria-based referrals within 8 working hours. Tracking patient referrals, diagnostics, and follow-ups to ensure smooth transitions. Facilitating MDT meetings by preparing patient notes and recording outcomes. Updating patient records with MDT outcomes and communicating with healthcare partners. Assessing patient needs for support services and making referrals. Monitoring patient care pathways, post-discharge follow-up, and completing data reports. Educating patients about care options and providing emotional support to navigate their journey. Prioritizing workload and developing systems to enhance effectiveness.

Requirements

  • Diploma or degree in Nursing, Physiotherapy, Clinical Associates, Paramedics, Critical Care Assistants, Emergency Care Practitioners, or relevant qualifications covering Anatomy, Physiology, Pathophysiology, and Microbiology. Minimum 3-5 years' clinical experience as a registered healthcare worker.
  • Current SANC / HPCSA registration or relevant medical qualification registration.
  • Knowledge of clinical coding and Life Case Management Toolkit is advantageous.
  • Renal knowledge is an advantage.
  • Knowledge of Renal Medical Aid billing processes is an advantage.
  • Understanding of CPT / CCSA & ICD coding is advantageous.
  • Familiarity with funder contracts, scheme rules, exclusions, and benefits is advantageous.
  • Knowledge of hospital industry practices.
  • Computer proficiency.
