Key Responsibilities:

Possess in-depth knowledge and comprehension of the Aged Care Quality Standards, Aged Care Legislation, and related principles.

Exhibit a strong commitment to providing person-centred support to clients.

Possess knowledge of the Aged Care sector, including Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP).

Deliver person-centred care coordination to consumers and assist in the day-to-day operations of Care for You at Home.

Undertake initial intake assessments.

Maintain accurate administration, including updating reports and government portals.

Ensure compliance with relevant regulations and best practices.

Perform additional responsibilities as outlined in the position description.

Familiarity with the NDIS Quality Standards is desirable.

Requirements:

Diploma or Cert IV qualification in Health, Community Services, Welfare, or Social Science.

Working with Children Check

Current driver's license and access to a comprehensively insured vehicle.

Proven experience in community care operations within a healthcare setting.

Ability to multitask and prioritise workload effectively with effective communication skills.

What we offer:

Permanent Full-time role

Working Hours: Monday to Friday, 08:30 am – 04:30 pm (Flexible start and end times).
