Program Manager - Community Case Management

The Program Manager has clinical, administrative, and supervisory responsibility for the implementation and operation of the Core Case Management team. This role works closely with Team Leads to achieve goals and objectives within program time limitations.

In this capacity, the Program Manager is responsible for implementing effective case management practices to meet the needs of diverse clients. They must possess excellent leadership and communication skills to guide a team of professionals in delivering high-quality services.

Key Responsibilities
  • Implement and operate the Core Case Management team.
  • Work closely with Team Leads to ensure achievement of program goals and objectives.
  • Schedule and conduct regular team meetings to discuss ongoing projects and provide feedback.
Required Qualifications
  • Master's degree in social work or a related field with at least five years of experience in mental health and/or social services.
  • Certification as a Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) is required; a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) is strongly preferred.
